• Entry step (January 2009): Required -- Select "Basic Entry/Exit"
• DHS Youth Step 1 Assessment (September 2009)
• DHS Youth Outcomes Assessment (October 2009)
• DHS Youth Program Exit (September 2009)

Data entry forms have been provided for your convenience. These forms are in Microsoft Word format so you may print or save them to your computer. You may reformat these forms to better suit your agencies needs.

DHS Youth reports are due quarterly and can be found in the DHS Youth folder and the Barriers Report folder in the Advanced Reporting Tool (ART).
