ServicePoint assessments and data entry forms

ESGP requires you to complete the following portions of ServicePoint:

• Universal Profile (June 2010)
• Household Information Form (June 2010)
• ESGP All-Inclusive Assessment (June 2010): Complete for single clients, head of household, and all adults 18+ in the household
• Service transactions (April 2009)
• Note: Entry/Exits are not required for ESGP

Data entry forms have been provided for your convenience. These forms are in Microsoft Word format so you may print or save them to your computer. You may reformat these forms to better suit your agency's needs.

• Client Consent Forms (December 2009)

ESGP reports due quarterly and can be found in the ESGP folder in the Advanced Reporting Tool (ART).

• ESGP Counting

ESGP reports are cumulative. The start date for ESGP reports is always July 1st.
Fall: July 1-September 30
Winter: July 1-December 31
Spring: July 1-March 31
Summer: July 1-June 30
