ESP requires you to complete the following portions of ServicePoint:

• Universal Profile (June 2010)
• Household Information Form (June 2010)
• ESP All-Inclusive Assessment (June 2010) Complete for single clients, head of household, and all adults 18+ in the household
• Service transaction (April 2009)
• Note: entry exit step NOT required

Data entry forms have been provided for your convenience. These forms are in Microsoft Word format so you may print or save them to your computer. You may reformat these forms to better suit your agency's needs.

• Client Consent Forms (December 2009)

ESP reports due semi-annually and can be found in the ESP folder in the Advanced Reporting Tool (ART).

ESP reports are cumulative. The start date for ESP reports is always July 1st.
