These forms are in Microsoft Word format so you may print or save them to your computer. You may reformat these forms to better suit your agency's needs.

Reports	HPRP reports are due quarterly and can be found in the HPRP folder in the Advanced Reporting Tool (ART). HPRP Counting Report QPR Grant start date is always 10/01/2009.
