Reports due to Mahube Community Council:

  • LTH Demographic/Entry Data Report due monthly, on the 10th of each month
  • All other LTH reports (those due quarterly to the state) due to Mahube two weeks before they are due to the state.

ServicePoint assessments and data entry forms LTH requires you to complete the following portions of ServicePoint:
* If you get money through the DHS Program as a subgrantee of Hearth Connection, consult with Hearth Connection about your data entry procedure.

Data entry forms have been provided for your convenience. These forms are in Microsoft Word format so you may print or save them to your computer. You may reformat these forms to better suit your agency's needs.

LTH reports are due quarterly and can be found in the LTH folder and the Barriers Report folder in the Advanced Reporting Tool (ART).
