Consent forms
- Clients MUST be given a "Data Privacy Notice and Consent" (consent) form that explains the Homeless Management Information System (HMIS) and why their personal information is being collected. It is helpful to walk through the consent with clients to ensure they have read and understand the form.
- The "Data Privacy Notice and Consent" (consent) form must be given to EACH adult and unaccompanied youth. Parents/guardians should sign for their children.
- If any client does not sign the "Data Privacy Notice and Consent" (consent) form (or one is not signed on their behalf), that client must be entered into ServicePoint as an anonymous client. Please see instructions on entering client anonymously.
- IF the client record is to be shared with other agencies, clients must also sign a "Release of Information" (ROI) form. It is helpful to walk through the ROI with clients to ensure they have read and understand the form.
- Both "Data Privacy Notice and Consent" (consent) form and the "Release of Information" (ROI) form are designed to meet legal requirements and to provide clear information to the clients, seek legal counsel before modifying them in any way.
