FHPAP Program-Specific Data Element Changes

Due to the introduction of Fast Track funding, there have been recent changes to FHPAP program specific data elements in HMIS. This includes the addition of two new data elements, as well as changes to existing data elements. Read on for more details, including resources to help you complete these new data elements.

Recent FHPAP Payment

This data element is required at project entry for Heads of Household enrolled in your FHPAP project from June 1st, 2023 onward.

Why do I need to answer this question? To confirm whether or not agencies have checked a client's record in HMIS for recent payments (within the last 60 days) made on a client’s behalf to avoid duplicate payments to a landlord.

Data Collection Instructions: At project entry, check within the client’s record for recent FHPAP payments. For instructions on where to check for payments within the client’s record, review the Checking for Recent FHPAP Payments Knowledge Base article. Record this data using the dropdown menu.

FHPAP Funding Sub-Assessment

This data element is required for Heads of Household with an open enrollment in an FHPAP project June 1st, 2023 and onward.

 

Why do I need to track this information? The FHPAP funding sub-assessment was created to help agencies track which FHPAP funding (Standard or Fast Track) is associated with a client throughout their time in your program. Starting June 1st, 2023, all Heads of Household active in your FHPAP project need to have the FHPAP funding sub-assessment completed in order to pull correctly into reports.

Data Collection Instructions: Starting June 1, 2023, and onward, the FHPAP Funding sub-assessment records must cover the entire time a Head of Household is enrolled in your FHPAP project. If funding changes within the client’s enrollment in your FHPAP project, you should update this information via Interim Review. Service Transaction Funding Source Updates

"FHPAP Fast Track" has been added to the list of available funding sources for Service Transactions. You will use this option for any direct assistance payments utilizing Fast Track funds. Continue to use "FHPAP" for direct assistance paid with Standard FHPAP funds. M-PAT

As of June 2023, MN Housing no longer requires the MN: Homeless Prevention Assessment Tool (M-PAT) to be completed in HMIS. Your regional grantee might still require this assessment to be completed for clients that are eligible for FHPAP assistance in Prevention programs, and in some cases for Doubled-up and Outreach providers. Providers should reach out to their regional grantees if they have specific questions.
