Looking for Coordinated Entry Forms? 

Coordinated Entry (CE) data collection forms are not managed centrally by your HMIS lead. They are created and maintained locally by individual Continuums of Care (CoCs). If you or your team are looking for your region's CE forms, please reach out directly to your local Priority List Manager.
