What’s New?
We heard your feedback and redesigned these optional forms to match the natural workflow in HMIS. Our goal is to save you time and reduce confusion.
How the New Forms Are Organized
The page is now broken down into clear, sequential steps to match a client's journey:
New Client Form: Use this to create or update a client record in HMIS. Tip: Print off one form for each individual household member.
Entry Forms: Captures the basic data elements needed to start a project entry. Available in separate formats for a Single Individual or a full Household.
Update/Annual Forms: Built specifically for tracking changes during a client's stay or completing required annual assessments. Available for both Singles and Households.
Exit Forms: Everything you need to properly close out a client's enrollment in your project. Available for both Singles and Households.
A Quick Note on Funder-Specific Forms
These core forms gather the universal and common data elements required across the board. If your project has specialized funding, you will use these core forms in partnership with the Funder Specific Data Collection forms.
Remember: If a client is served by multiple grants, you’ll just need to print out each specific program’s form separately to catch those extra required data fields.
Looking for Coordinated Entry Forms?
Please note that Coordinated Entry (CE) data collection forms are not managed centrally on this page. They are created and maintained locally by individual Continuums of Care (CoCs). If you or your team are looking for your region's CE forms, please reach out directly to your local Priority List Manager.
