Care Coordination
Below are instructions to access the Care Coordination assessment in HMIS. The Care Coordination assessment, combined with the other information on the Summary tab in HMIS, brings together information from a variety of places in HMIS to show an up-to-date "snapshot" of a client's HMIS record. This can help HMIS users across the continuum who work with an individual to see and update critical information that can be used to connect someone with needed resources.
