Designated Agency Contacts Outreach

ICA has updated our Agency Agreement, and is asking all partner agencies to sign the revised document, along with providing designated contact information. ICA is reaching out to all HMIS agencies for a bit of information, and we would like your help.  

Since Minnesota's HMIS moved to a new software system in 2024, we are asking all agencies to complete a new Agency Agreement. This form underwrites the legal relationship between an agency and the State System Administrator as it relates to HMIS responsibilities. This document should be carefully reviewed and signed through signNow by the agency’s executive director. Additionally, we are asking our partners to verify agency contacts with decision-making permissions related to HMIS, licenses and billing, and data quality. 

Along with the new signed Agency Agreement, please respond with the names and emails for all contacts within the next two months from the date of the first email. The contact information does not have to be three different people.  

The roles we are looking to identify are: 

• Designated Agency Contact – Provide updates to Lead Agency and respond to inquiries from Lead Agency. 

• Billing Contact – Has the authority and ability to pay bills. 

• Data Quality (DQ) Contact – Review the agency’s HMIS data quality regularly.
